You have been invited to take part in an audit which will capture how Health and Social Care staff are providing care to people who are dying and those who have been bereaved.
You will have experienced a personal bereavement or are a professional who provides care for dying and bereaved people. Professionals can tell personal as well as work related stories.
The results will be used by care providers to inform future services.
You will be required to type your bereavement story/experience directly onto the computer and answer a few questions to help with analysis.
When you reach the end you will have the opportunity to return to this screen and repeat the process for another story.
